Enrollment, step by step

Set up in about 15 minutes

From hire date to active coverage — here is what to expect. Save your confirmation at the end; that is your receipt.

  1. Log Into ADP Self-Service

    Access the ADP Self-Service portal to view available benefit options and make your elections.

  2. Review Your Options

    Compare the $1,500 Copay Plan and $5,000 HSA Plan for medical. Review dental, vision, voluntary benefits, and financial benefits including HSA, FSA, and 401(k).

  3. Add Dependents

    Add eligible dependents (spouse, domestic partner, children) to your benefit elections. Have dependent information (date of birth, SSN) ready.

  4. Submit Elections

    Confirm and submit your benefit elections within the enrollment window. Elections are irrevocable until the next open enrollment unless you have a qualifying life event.

  5. Confirm Enrollment

    Review your confirmation statement and keep a copy for your records. Contact HR at 952.283.2061 with any questions.

Class 1 — Regular Full-Time

Medical Premium Contribution

Employer-Paid

~54% paid by Integreon Global

Integreon Global covers the majority of your medical insurance premiums — saving you hundreds per month compared to individual market rates.

HSA Employer Contribution

Employer-Paid

$500 – $1,000 / year

If you enroll in the $5,000 HSA Plan, Integreon Global contributes $500 (employee only) or $1,000 (with dependents) to your Health Savings Account — deposited in January and July.

Basic Life & AD&D Insurance

Employer-Paid

1× salary up to $50,000

Employer-paid basic life insurance and accidental death & dismemberment coverage equal to 1× your annual salary, up to $50,000.

Short-Term Disability

Employer-Paid

60% pay, up to $1,000/week

Employer-paid STD coverage replaces 60% of your base salary if you are unable to work due to illness or injury, up to $1,000/week.

Long-Term Disability

Employer-Paid

60% pay, up to $10,000/month

Employer-paid LTD coverage replaces 60% of your base salary for long-term disabilities after the STD period ends, up to $10,000/month.

401(k) Safe Harbor Contribution

Employer-Paid

3% of salary (immediate vest)

Integreon Global contributes 3% of your eligible compensation to your Fidelity 401(k) as a safe harbor contribution — 100% vested from day one.

Employee Assistance Program

Employer-Paid

3 in-person + 5 telehealth visits/year

ComPsych EAP is fully employer-paid and available from date of hire. Covers counseling, legal consultations, financial guidance, and caregiving support at no cost to you.
